While you are signed in, the Help / Report button is in the tools panel that opens from the chevron on the right-hand rail of the screen, and the same Help / Report row is in Settings. Choose Report a problem or Request a feature, describe what happened, and send it.
A player can be reported from the report option in chat (Report this player). Rules and consequences are in the community rules.
